Dental Forms

All current second grade students must show proof of a dental examination by May 15, 2010. If your second grade student has been to the dentist anytime since Nov 15, 2008, please take your dental form to your dentist and have it filled out, then return the form to your school's Health Office by May 15, 2010. All first and fifth grade students may also turn in their dental forms at this time and it will apply towards the 2010-2011 school year.

Dental exam forms can be found on the District 57 web site at www.d57.org, click on Resources, then Health Information. See Dental Forms on the right side of the screen under Downloadable forms. Dental forms are also available at any District 57 School Health Office.
