District 57 Fee Waiver Programs
Lunch/Milk Fees:
Mount Prospect School District 57 participates in the National School Lunch/Illinois Free Lunch Program to support families requesting assistance in providing an adequate nutritious lunch and/or milk for their students and for families meeting the federal qualifying income level guidelines. This program is separate from the student fee waiver program and requires a separate application.

Student Fees:
Mount Prospect School District 57 uses the free meals qualifying income level guidelines for the National School Lunch Program to determine if a student qualifies for a fee waiver. By meeting the qualifying income level guidelines and verification process pursuant to Board Policy 4:140, the following fees are waived: instructional, recorders, locks, middle school gym suit, extracurricular activities, graduation, and field trips including transportation (when an alternative curriculum is not offered). Additionally, fees for student participation in the Kids’ Corner Program are reduced by 50%. Transportation fees in accordance with Board Policy 4:110 are not waived.

Submit the completed fee waiver form along with a copy of income tax forms (1040 and W-2), a copy of current pay stub for all family members, and any other forms required per your application to the District 57 Business Office:
