In January 2008, a state law was passed (Public Act 95-671) requiring that all children enrolling in kindergarten in a public, private, or parochial school shall receive an eye examination. This examination must be by an optometrist or physician who provides complete eye examinations. According to the law, this exam must be submitted to school no later than October 15, 2008. A hard copy of the Illinois Department of Public Health Eye Examination Form and instructions will be available with registration packets.

The Illinois Department of Public Health requires a dental examination of all children in kindergarten, second and sixth grades. The authorized form must be completed and submitted to the school health office by May 15th of the child’s kindergarten, second or sixth grade school year. Exams completed up to 18 months prior to the May 15th deadline will be accepted. A hard copy of the Illinois Department of Public Health Dental Examination Form and instructions will be sent home at the start of the 18 month time frame.
